SLP Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate and treat speech, language, and swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continually assess and revise care plans with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote independence.
• Document care observations using a touch pad tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in VA.
• ASHA CCC-SLP or eligibility for certification, or equivalent.
• Educational requirements to qualify for CCC-SLP, and/or supervised experience in progress.
• Solid computer skills; experience with EMR preferred.
