SLP Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations to diagnose and provide speech, language, or swallowing interventions.
• Develop a treatment plan based on evaluation and goals.
• Continually assess and revise the care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote independence.
• Document observations and interventions in patient records using EMR.

🎯 Requirements

• Current VA license as an SLP.
• ASHA CCC or equivalent certification requirements.
• Education and experience required for certification.
• In process of supervised experience for certification.
• Solid computer skills; EMR experience preferred.
