• Collaborate with Rehab Manager, Director, and DCO for cohesive management.
• Review clinical documentation for quality and accuracy.
• Ensure appropriate clinicians are assigned to each client.
• Update care plans based on ongoing assessments.
• Coordinate client care, referrals, and advocate for clients.

🎯 Requirements

• 2+ years as an RN in Home Health.
• 2+ years in a leadership/supervisory role.
• Excellent organization and communication skills.
• Desire to lead, inspire, and motivate others.
• Strong knowledge of home health policies and procedures.
