Make home visits to clients in designated territories.
Conduct evaluations to diagnose and treat speech, language, or swallowing issues.
Create treatment plans based on evals, orders, and goals.
Review and revise care plans with the multidisciplinary team.
Educate patients and families on rehabilitation and independence.
Document observations and interventions using an EMR or tablet.

🎯 Requirements

VT SLP license.
ASHA CCC or equivalent seeking certification.
In progress toward ASHA certification.
Completed education for certification.
Strong computer skills; EMR experience preferred.
