Make home visits to clients in designated territories.
Perform evaluations to diagnose and treat speech, language, or swallowing issues.
Develop a treatment plan based on evaluation, orders, and goals.
Continuously assess and revise care plans with the care team.
Educate patients and families on rehabilitative care to promote independence.
Document care with a touchpad tablet.

🎯 Requirements

Current Vermont SLP license.
ASHA CCC-SLP or equivalent certification.
Solid computer skills; EMR experience preferred.
Experience with adult/geriatric populations preferred.
Able to work in a multidisciplinary team.
