Make outpatient visits to clients in designated geographic territories.
Perform diagnostic tests and measurements: mobility, ROM, gait, strength, balance, safety.
Develop and implement individualized care plans with manual therapy, gait training.
Assess and revise PT care plans; collaborate with nursing.
Educate clients and family members on rehabilitative care and safety.
Document observations and care using a touch-pad tablet.

🎯 Requirements

Vermont PT license.
Minimum 1 year PT experience.
Graduate of APTA-approved program.
Ability to work independently.
Strong interpersonal and organizational skills.
EMR experience preferred.
