📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Develop and implement individualized care plans, including gait training and balance work.
• Revise care plans and collaborate with nursing in multidisciplinary team.
• Educate clients and families on rehab activities for health and independence.
• Document observations and interventions in the EMR with a touchpad tablet.

🎯 Requirements

• Vermont Physical Therapist license.
• Minimum 1 year of PT experience.
• Graduation from an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
