📋 Description

• Make outpatient visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Develop and adjust care plans with manual therapy and gait training.
• Coordinate with nursing on multidisciplinary care plan.
• Educate clients and families on home exercises and safety.
• Document observations and interventions on a touch-pad tablet.

🎯 Requirements

• Current New Hampshire PT license.
• Minimum of one year of PT experience.
• Graduate from APTA-approved program or AMA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
