📋 Description

• Make outpatient visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, transfers, balance and safety.
• Develop individualized care plans (manual therapy, gait training, balance).
• Continually assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families on rehabilitative care and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• A current New Hampshire Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
