📋 Description

• Serve as primary ongoing contact for Medicare patients.
\n
• Build trusting relationships grounded in empathy and follow-through.
\n
• Help patients understand care plans, appointments, benefits, resources.
\n
• Conduct person-centered assessments of life context, goals, barriers.
\n
• Perform SDOH risk assessments and identify unmet social needs.
\n
• Develop action plans aligned to initiating-visit problems or high-risk conditions.
\n

🎯 Requirements

\n
• 3+ years in patient advocacy, care coordination, or healthcare navigation.
\n
• Strong understanding of the US healthcare system, especially Medicare.
\n
• Experience with older adults or medically complex populations.
\n
• Excellent verbal, written, and interpersonal communication.
\n
• Comfortable using EHRs, CRMs, and patient messaging tools.
\n
• Ability to work independently in a remote environment.
\n
