Lead, inspire and motivate others to provide exceptional care.
Collaborate with Rehab Manager, Director, and DCO to manage field staff.
Review clinical documentation to ensure high-quality services.
Ensure the right clinicians are assigned to each client.
Maintain and update each client's care plan based on ongoing assessment.
Coordinate client care, referrals and act as internal case manager and advocate.

🎯 Requirements

2+ years RN experience in Home Health.
Oasis experience required.
2+ years in leadership/supervisory role.
Excellent organization and communication skills.
Desire to lead, inspire and motivate others.
Strong knowledge of home health policies and procedures.
