RN Nurse Manager, Home Health

📋 Description

• Lead, inspire and motivate staff to provide exceptional care.
• Collaborate with Rehab Manager, Director, and DCO to manage field staff.
• Review clinical documentation for consistency and quality.
• Assign clinicians to clients to ensure appropriate care.
• Develop and update each client’s care plan based on ongoing assessment.
• Coordinate care, referrals, and act as internal case manager and advocate.

🎯 Requirements

• Active RN license in state.
• 2+ years RN experience.
• Acute/Med-Surg or sub-acute experience.
• Supervisory leadership experience.
• Knowledge of home health and Medicare certs.
• Strong interpersonal and leadership skills.
