RN Navigator - CKD

📋 Description

• Care coordination, education, and support for CKD patients.
• Liaise with patients, families, nephrologists, and care team.
• Drive transitions of care and promote self-management.
• Use an integrated tech platform with the interdisciplinary team.
• Lead on identifying care gaps and CKD to ESKD planning.
• Coordinate transplant eval, dialysis access, and home dialysis training.

🎯 Requirements

• Registered Nurse with active state licensure.
• BSN preferred.
• 5+ years experience; 3+ in nephrology/dialysis/transplant.
• Strong communication and teaching skills.
• Experience with care coordination and patient education.
• Able to work effectively in a remote environment.
