Follow a designated care plan in accordance with patient’s needs
Make home visits to clients in designated geographic territories
Administer medications, wound care, treatments, and procedures
Monitor clients' conditions and report changes to the Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments and care management

🎯 Requirements

A current PA RN license
At least 1 year of recent clinical (medical/surgical) experience
Graduation from accredited nursing program
Ability to work independently and manage time
Strong interpersonal skills
Solid computer skills; EMR experience preferred
Prior home care experience preferred but not required
