- Direct and coordinate care with a multi-disciplinary home health team
- Process incoming referrals and manage caseload
- Mentor Clinical Associates and field staff
- Onboard and orient new field staff
- Review OASIS and HCHB documentation
- Monitor quality assurance and staff productivity
- This is a full-time in-office position with client visits as needed
🎯 Requirements
- Active Colorado or Compact RN license
- Medicare-certified Home Health Care experience
- Knowledge of Medicare Conditions of Participation
- Proficiency with OASIS documentation and ICD-10 coding
- HCHB experience a plus
- Strong leadership, organizational and communication skills
