RN Clinical Manager, Home Health

📋 Description

• Lead and coordinate care by a multi-disciplinary home health team.
• Process incoming referrals.
• Mentor Clinical Associates and field staff.
• Onboard and orient new field staff.
• Review OASIS and HCHB documentation.
• Monitor quality assurance.
• Oversee staff productivity and case management.
• In office role with client visits as needed.

🎯 Requirements

• Active Hawaii RN license.
• Home Health experience preferred.
• Medicare Certified Home Health Care experience.
• Knowledge of Medicare Conditions of Participation.
• OASIS documentation and ICD-10 coding experience.
• HCHB knowledge a plus.
• Strong people management and leadership skills.
• Excellent organizational and communication skills.
• Acute/med-surg/sub-acute experience preferred/required.
