RN Clinical Manager - Home Health

📋 Description

• Lead, inspire and motivate others to provide exceptional care.
• Work with Rehab Manager, Director, and DCO for cohesive field staff.
• Review clinical documentation for consistent, high-quality services.
• Collaborate with office team to assign the most appropriate clinicians.
• Develop and update the client's plan of care based on assessments.
• Coordinate client care and referrals; act as internal case manager.

🎯 Requirements

• 2 or more years of RN experience in Home Health.
• OASIS experience required.
• 2+ years in a leadership/supervisory role.
• Excellent organization and communication skills.
• Desire to lead, inspire, and motivate others.
• Strong knowledge of home health policies and procedures.
