RN Clinical Manager, Home Care

📋 Description

• Lead field staff to deliver client service, clinical care, quality, and compliance.
• Conduct admission visits, clinical assessments, and care planning.
• Serve as internal case manager and advocate with clients, families, and care teams.
• Review clinical documentation for accuracy, timeliness, and policy compliance.
• Develop and update individualized plans of care based on ongoing assessment.
• Provide clinical supervision, coaching, and performance feedback to field staff.

🎯 Requirements

• Current MO RN license in good standing.
• Graduate of an accredited nursing program.
• Minimum two years of related clinical experience; home care or case management.
• Prior supervisory experience with staff development.
• Strong interpersonal, communication, leadership, and organizational skills.
• English: read, write, and communicate effectively; basic PC skills.
