📋 Description

• Lead, inspire and motivate field staff to deliver exceptional care.
• Coordinate client care, referrals, and serve as internal case manager.
• Review clinical documentation for consistent, high-quality services.
• Assign clinicians to clients and ensure appropriate care plans.
• Develop, implement and update client plans of care based on assessments.
• Represent BAYADA in community and marketing activities.

🎯 Requirements

• 2+ years RN Home Health experience.
• 2+ years leadership/supervisory experience.
• Strong organization and communication skills.
• Desire to lead, inspire and motivate others.
• Knowledge of home health policies and procedures.
• Experience coordinating care and developing care plans.
