📋 Description

• Lead, inspire, and motivate field staff to deliver exceptional care.
• Collaborate with Rehab Manager, Director, and DCO for cohesive staff management.
• Review clinical documentation for accuracy and quality of care.
• Assign clinicians to clients based on needs and skills.
• Develop and update the client's plan of care after assessments.
• Coordinate care, referrals, and advocate for clients and staff.

🎯 Requirements

• Active RN license in good standing.
• 2+ years RN Home Health experience.
• 2+ years leadership/supervisory role.
• Excellent organization and communication skills.
• Desire to lead, inspire, and motivate others.
• Strong knowledge of home health policies and procedures.
