RN Clinical Manager

📋 Description

• Direct care coordination by a multidisciplinary home health team.
• Process incoming referrals.
• Manage and mentor Clinical Associates and field staff.
• Orientation of field staff.
• Documentation review, including OASIS and HCHB.
• Monitor quality assurance and overall case management; visit clients as needed.

🎯 Requirements

• Active Hawaii RN license.
• At least 2 years nursing experience.
• Recent Medicare-certified Home Health Care exp.
• Knowledge of Medicare Conditions of Participation.
• Thorough understanding of OASIS documentation and ICD-10 coding.
• HCHB knowledge a plus.
