RN Clinical Manager

📋 Description

• Supervise and mentor RNs, RN Designees, LPNs, and home health staff
• Provide clinical leadership for complex cases and urgent situations
• Complete initial patient assessments and develop individualized care plans
• Use the RN Designee visit team for supervisory visits enabling safe, family-centered care
• Partner with physicians, therapists, schools, and families to coordinate care
• Collaborate with intake and scheduling teams to ensure smooth transitions of care

🎯 Requirements

• Active RN license in New York State (required)
• 2+ years of Registered Nurse experience (home care or pediatrics preferred)
• Trach and vent experience or strong willingness to learn
• Strong communication, leadership, and mentoring skills
• Willingness to travel locally for supervisory visits; virtual visits after 6 months
• Ability to travel within Westchester, Orange, and Rockland Counties
