RN Clinical Care Navigator- Hospice

📋 Description

• Liaison and educator on hospice, home health, and palliative care.
• Conduct goals of care conversations with patients.
• Coordinate transfer of high-risk hospice patients from hospital to home.
• Provide direct patient care to inpatient hospice when needed.
• Support families and care teams to ensure seamless care.

🎯 Requirements

• Must be a registered nurse (RN) with MO licensure.
• CHPN certification preferred.
• Minimum 1 year hospice experience.
• Experience in care management and discharge planning preferred.
• Ability to read, write, and communicate in English.
• Pre-employment assessment results acceptable.
