RN Care Manager (Travel)

📋 Description

• Provide direct nursing care in clinic, home, or SNF.
• Serve as quarterback of the Interdisciplinary Team (IDT) for care plans.
• Assess participants and drive care plans; coordinate across settings.
• Conduct nursing assessments and contribute to care plan updates.
• Triage physician orders; document meds promptly.
• Support transitions of care and coordinate with hospitals and LTC partners.

🎯 Requirements

• Graduate of an accredited nursing program; BSN preferred.
• Unencumbered RN license; state licensure as required.
• Minimum 2 years' clinical experience with medically complex/older adults.
• Wound care, IV therapy, phlebotomy, meds within RN scope.
• CPR/BLS current; Alzheimer's cert preferred; bilingual a plus.
• Valid CA driver’s license with reliable transportation; travel up to 50%.
