RN Care Manager (PACE)

📋 Description

• Provide direct nursing care across clinic, home, and other sites.
• Act as quarterback for your panel within the Interdisciplinary Team (IDT).
• Assess participants and drive individualized care plans.
• Contribute to a center experience that fosters belonging and joy.
• Continuously raise the bar by sharing feedback and implementing changes.
• Coordinate care transitions, hospital or long-term care referrals.

🎯 Requirements

• Graduate of an accredited school of nursing; BSN preferred.
• Unencumbered RN license (CA license to practice).
• Minimum 2-4 years nursing experience with medically complex or older adults.
• Strong clinical acumen in chronic disease and geriatric care.
• Alzheimer’s certification and Case Management certification preferred.
• CPR/BLS current; immunizations and TB test required.
