RN Care Manager

📋 Description

• Provide direct nursing care to participants in clinic, home, or center.
• Serve as quarterback for IDT, driving assessments and care plans.
• Conduct assessments and contribute to care plans and coordination.
• Coordinate with teams to ensure smooth transitions and outcomes.
• Support care coordination and education for participants and families.
• Participate in quality improvement and patient experience initiatives.

🎯 Requirements

• Graduate of accredited nursing program; BSN preferred.
• Unencumbered RN license.
• 2-4 years RN experience with medically complex or older adults.
• 1-2 years case management experience.
• CPR/BLS certification required or obtain within 30 days.
• Valid CA driver's license with good driving record and auto insurance.
