RN Care Manager

📋 Description

• Provide direct nursing care to participants in clinic, adult day center, home, virtually, or SNF.
• Act as quarterback for your panel within the Interdisciplinary Team to drive assessments and care plans.
• Conduct comprehensive nursing assessments across settings (clinic, in-home, etc.).
• Deliver and document nursing interventions; administer medications and therapies per care plans.
• Coordinate care delivery including medication management, equipment, supplies, and referrals.
• Participate in IDT meetings and educate participants and caregivers about care plans.

🎯 Requirements

• Graduate of an accredited school of nursing; BSN preferred.
• Unencumbered RN license.
• Minimum 2-4 years of clinical experience caring for medically complex or older adults.
• Minimum 1-2 years experience in case management.
• Alzheimer’s certification and case management certification preferred; bilingual preferred.
• Current CPR/BLS certification; up-to-date immunizations and TB test; valid CA driver's license with good driving record and auto insurance.
