• Support CCM care for CKD, HTN, diabetes, and CVD.

• Salary: $45,000-$52,000 annually.

🎯 Requirements

• Certified Medical Assistant
• 1–3 years healthcare experience
• Chronic disease or nephrology experience preferred
• Strong clinical assessment, communication, documentation
• Two systems: EHR + CCM platform
• MS Office Suite including Outlook and Teams
