Remote Licensed Practical Nurse - Mountain Time

📋 Description

• Conduct monthly CCM check-ins; address clinical concerns
• Reinforce care plans; ensure accurate EHR/ CCM documentation
• Collaborate with providers; escalate issues per triage pathways
• Use SMS as primary engagement channel; phone outreach when needed
• Perform structured symptom assessments; evaluate condition changes
• Coordinate follow-ups, labs, and nephrology appointments

🎯 Requirements

• Active LPN or LVN license
• 1-2 years clinical experience in ambulatory care, population health, care coordination, or case management
• Strong clinical assessment, communication, and documentation skills
• Comfortable using two systems: EHR and CCM platform
• Ability to manage a structured, metric-driven workflow
• Proficient in MS Office (Outlook and Teams) and remote work setup
