🎯 Requirements

• 3+ years in health care coordination or similar field.
• Credentialed Community Health Worker.
• Experience with team-based care delivery.
• Excellent knowledge of community resources.
• Strong communication and teaching skills; MS Office proficiency.
• Ability to work remotely with reliable internet and dedicated workspace.
