Provide medication management including reconciliation, dose optimization, adherence counseling.
Collaborate with the interdisciplinary care team to develop individualized care plans per guidelines.
Monitor and evaluate medication regimens for therapeutic goals, adverse effects, and interactions.
Identify opportunities for drug therapy optimization and cost savings.
Participate in quality improvement, clinical outcomes, REMs compliance, and ACP projects.
Support workflow optimization, including prior authorizations with clinical justification.

🎯 Requirements

PharmD and CA Pharmacist license required.
Advanced practice pharmacist in CA: two of three criteria.
Postgraduate residency (PGY1/PGY2) preferred.
3+ years' experience as a clinical pharmacist.
Strong knowledge of pharmacology and FDA health regs.
Excellent verbal and written communication.
