Remote Ambulatory Care Pharmacist

📋 Description

• Provide medication management, reconciliation, and adherence counseling per protocol.
• Collaborate with interdisciplinary care team to develop individualized therapy plans.
• Monitor and optimize medication regimens to meet therapeutic goals.
• Identify opportunities for drug therapy optimization and cost savings.
• Support workflow improvements and coordination of care across departments.
• Advocate for patient safety and medication literacy through education.

🎯 Requirements

• PharmD and CA pharmacist license.
• Advanced practice CA pharmacist criteria: two of: certification, residency, or 1,500 hours.
• 3+ years clinical pharmacist experience; PGY1/PGY2 preferred.
• Strong pharmacology knowledge and familiarity with FDA regulations.
• Experience with disease-state management and therapy optimization.
• Excellent verbal communication, analytical skills, and teamwork.
• Highly organized, detail-oriented, and maintains confidentiality.
