- Monitor patient status; report changes to Clinical or Client Services Manager

- May assist with other visit types as needed
🎯 Requirements
- Current Colorado RN license
- Prior home health/visits experience
- Experience completing OASIS documentation
- Weekend Start of Care (SOC)/Admissions experience preferred
- Medicare Certified Home Health knowledge
- Graduation from an accredited nursing program
