Make home visits to clients in designated geographic territories.
Administer medications, wound care, treatments, and comfort care.
Monitor client conditions and report changes to Clinical or Client Services Manager.
Follow up with, execute, and properly document doctors' orders.
Perform client assessments as necessary.
On call after hours on weekends.

🎯 Requirements

Current VA RN license.
Minimum 1 year clinical (medical/surgical) experience.
Prior home care/hospice experience preferred, not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
Availability for on-call and after-hours visits.
