Follow a designated care plan in line with patient needs
Make home visits to clients in designated geographic territories
Administer meds, perform wound care, treatments and procedures
Monitor client conditions and report changes to Clinical or Client Services Manager
Follow up with and document doctors' orders
Perform client assessments as necessary

🎯 Requirements

Current RN license in Pennsylvania
At least one year of clinical (medical/surgical) experience
Graduation from an accredited nursing program
Ability to work independently and manage time effectively
Strong interpersonal skills
Solid computer skills with EMR experience preferred
