- Follow a designated care plan in line with patient needs
- Make home visits to clients in designated geographic territories
- Administer meds, perform wound care, treatments and procedures
- Monitor client conditions and report changes to Clinical or Client Services Manager
- Follow up with and document doctors' orders
- Perform client assessments as necessary
π― Requirements
- Current RN license in Pennsylvania
- At least one year of clinical (medical/surgical) experience
- Graduation from an accredited nursing program
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills with EMR experience preferred
