📋 Description

• Perform head-to-toe assessments to identify subtle patient changes.
• Utilize experience with acute and post-acute care (CHF, COPD, wound care, IV therapy).
• Triage and manage urgent clinical situations independently at home.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

🎯 Requirements

• Current nursing license in good standing (MA).
• Graduate from an accredited nursing program with transcript or diploma.
• At least 1 year of adult/geriatric clinical experience in past 2 years (acute care or home health).
