- Making after-hours home visits in designated territories.
- Administering medications, wound care, treatments, and comfort care.
- Monitor client conditions; report changes to Clinical/Client Services Manager.
- Follow up on and document doctors' orders.
- Perform client assessments as needed.
- Occasional on-call after hours and/or weekends.
🎯 Requirements
- A current license as a Registered Nurse (RN) in VT.
- A minimum of one year of recent clinical (medical/surgical) experience.
- Prior home care/hospice experience preferred but not required.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
