Making home visits to clients in designated geographic territories.
Performing admin of medication, wound care, treatments and comfort care.
Monitoring clients' conditions; report changes to Clinical or Client Services Manager.
Following up with, executing, and documenting doctors' orders.
Performing client assessments as necessary.
Occasional on-call after hours and weekends.

🎯 Requirements

A current PA RN license.
Minimum 1 year of recent clinical (medical/surgical) experience.
Home health and/or hospice experience preferred, but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to assigned cases.
