- Making home visits to clients in designated geographic territories.

- Monitor client conditions and report changes to the Clinical/Client Services Manager.
- Follow up with, execute, and document doctors' orders.

- Occasional on call after hours and/or weekends.
🎯 Requirements
- Current RN license in VT and NH.
- Minimum of 1 year recent clinical (medical/surgical) experience.
- Prior home care or hospice experience preferred but not required.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
