- Make home visits to clients in designated geographic territories.
- Administer medications, provide wound care, treatments, and comfort care.
- Monitor client conditions and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and document doctor orders accurately.
- Perform client assessments as needed.
- Occasional on call after hours and weekends.
🎯 Requirements
- A current license as a Registered Nurse (RN) in VT and NH.
- Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred, but not required.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
