- Making home visits to clients in designated geographic territories.
- Administering meds, wound care, treatments, and comfort care.
- Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.

- Occasional on call after hours and/or weekends.
🎯 Requirements
- Current RN license in VT.
- Minimum 1 year of clinical (medical/surgical) experience.
- Home care/hospice experience preferred, not required.
- Strong computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
