Registered Nurse, RN - Home Visits

📋 Description

• Provide skilled nursing and rehabilitative care at patients' homes.
• Conduct head-to-toe assessments and monitor changes.
• Manage acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and independently manage urgent clinical situations at home.
• Medication reconciliation, patient teaching, and safety monitoring.
• Wound assessment, care, and documentation.

🎯 Requirements

• Active Colorado nursing license in good standing.
• Graduation from an accredited nursing program (transcript/diploma).
• At least 1 year of hands-on adult/geriatric care in acute, home health, or hospice settings.
