Registered Nurse, RN, Home Health Visits (Med Surg + Wound Care)

📋 Description

• Follow a designated care plan per patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments and procedures.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Document observations and interventions on EMR/tablet.
• Case management and coordination of care.

🎯 Requirements

• Current OH RN license.
• 1+ year of Med/Surg clinical experience.
• Home health experience preferred.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time.
• Strong interpersonal skills and EMR experience.
