- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories

- Monitor client conditions and report changes to Clinical/Client Services Manager

- Perform client assessments as necessary
🎯 Requirements
- Current OH RN license
- Minimum one year of recent medical/surgical clinical experience
- Prior home care experience preferred, but not required
- Graduation from an accredited nursing program per transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
