Follow designated care plans per patient needs
Make home visits to clients in designated territories
Administer medications, wound care, and treatments
Monitor client conditions; report changes to Clinical or Client Services Manager
Document doctors' orders and client observations in EMR
Conduct client assessments and coordinate case management

🎯 Requirements

Active RN license in PA.
Minimum one year clinical (medical/surgical) experience.
Home care experience preferred, but not required.
Graduation from an accredited nursing program.
Ability to travel to cases as assigned.
Ability to work independently and manage time.
