Follow designated care plans per patient needs
Make home visits to clients in designated territories
Administer meds, wound care, treatments per orders
Monitor client conditions; report changes to Clinical or Client Services Manager
Document doctors' orders and interventions accurately
Perform client assessments and support case management

🎯 Requirements

Current RN license in MA
Minimum 1 year clinical (medical/surgical) experience
Home health experience preferred
Graduation from accredited nursing program
Ability to work independently and manage time
EMR experience preferred
