- Follow designated care plan in line with patient needs.

- Perform client assessments and coordinate case management as needed.
π― Requirements
- Current Delaware RN license.
- 1+ year of recent clinical (medical/surgical) experience.
- Home health experience preferred but not required.
- Graduate of an accredited nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills; EMR experience preferred.
