- Make home visits to clients after hours and on weekends.
- Administer medications, wound care, treatments, and comfort care.
- Monitor clients' conditions; report changes to Clinical or Client Services Manager.
- Follow up on, execute, and document doctors' orders.
- Perform client assessments as necessary.
- On-call rotating schedule: 5pm to 8:30am; one week on/off.
🎯 Requirements
- Current NJ RN license.
- At least 1 year of recent clinical (medical/surgical) experience.
- Prior home care experience preferred, not required.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
