- Home visits to clients in designated territories after hours.
- Administer medications, provide wound care and comfort care.
- Monitor client conditions and report changes to the supervisor.
- Follow up on and document doctors' orders.
- Conduct client assessments as needed.
- On-call rotating schedule: 5pm–8:30am, one week on/off.
🎯 Requirements
- Current PA RN license.
- Minimum one year of recent clinical (medical/surgical) experience.
- Prior home care and/or hospice experience preferred.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases.
