- Making home visits after hours and on weekends.
- Administer medications, wound care, and hospice treatments.
- Monitor client conditions; report changes to Clinical/Client Services Manager.
- Follow up on doctors' orders and document them accurately.

- On-call rotating schedule: 5pm–8:30am; one week on, one off.
🎯 Requirements
- Current VT and NH RN licenses in good standing.
- Minimum 1 year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
